SCN of HRSN
The mission of New York State is to protect and promote health for all, building on a foundation of health equity. The Office of Health Insurance Programs (OHIP) has established regional Social Care Networks (SCNs) across the state to ensure that the Health-Related Social Needs (HRSNs) of Medicaid Members are more consistently identified and addressed.
Food As Medicine
A program done in conjunction with the Buffalo Niagara Medical Campus (BNMC) and the Department of Health and Well-being. We believe that addressing food as a social determinant of health through public health food and nutrition programs, alongside community partners will bring us closer to achieving health equity.
Food and Community
The FnC program, created in 2023 by Cornell Cooperative Extension of Erie County (CCE) and LifeSource Systems Inc. Through their partnership, CCE and LSS have set out to celebrate and explore the intricate and multifaceted relationship between food and community. This ultimate goal of the program is to promote a healthier and more connected community.
Black Farmers United NYS
As a member of BFU-NYS, an organization dedicated to advocating for Black farmers the organization is represented by Pamela James at both state and national conferences to promote the nine solutions outlined by the organization. These solutions, including creating ownership opportunities, funding Black farm and food startups, supplying affordable land, supporting urban farms, committing to inclusion, awarding scholarships, capturing meaningful data, and further developing a robust plan of action, geared towards empowering Black farmers and ensuring their success in the agricultural industry. This dedication and leadership have, inspires and empowers others to join the mission of uplifting Black farmers across the country.
Farm to Table
Food for the Spirit and the Buffalo Public Schools to run farm to table programs at East High School in Buffalo, New York. The collaboration aimed to not only provide fresh, locally sourced food to the school cafeteria but also to educate students about sustainable farming practices, nutrition, and the importance of supporting local farmers. Through this program, students had the opportunity to learn about the journey of food from the farm to their plate, fostering a deeper understanding of the food system and its impact on their daily lives. This initiative not only promoted healthy eating habits but also empowered the students to make informed choices about the food they consume, while also supporting the local community and economy.
Climate Change Educators
LSS Directors play a crucial role in their partnership with Re-Tree WNY. They actively contribute to tree planting in low-income BIPOC neighborhoods, advocate for climate change awareness, educate the community on sustainable practices, and advocate for policy development. The team is dedicated to addressing environmental concerns along with issues of poverty, housing discrimination, and social determinants of health in Black communities. The LSS staff’s commitment is exemplified by their certification as tree and climate stewards, enabling them to drive meaningful change. Their involvement in national events, such as James, co-founder, participating in the Rally for Resilience in Washington, DC, demonstrates their dedication to advocating for critical legislative changes, such as the passage of the farm bill.
Finance Empowerment
Financial literacy covers a wide range of subjects to help individuals make informed and confident money decisions.
Financial Planning
Setting financial goals, net worth, and long-term planning through considering pensions, 401(k)s, IRAs, Social Security, and employer benefits. Also, learn investing tools such as stocks, bonds, mutual funds, ETFs, risk, and diversification. In addition, learn the value of insurance to cover health, life, auto, renters/homeowners, disability, and business.
Banking, Budgeting, Taxes, Credit, & Debt Mgt.
An exploration of checking and savings accounts, interest, fees, and digital banking tools. Learn about emergency funds, short-term vs. long-term savings, and goal-based saving Understand tax brackets, deductions, credits, pay stubs, credit scores, credit reports, loans, credit cards, and debt reduction strategies. Also, track income and expenses, creating budgets, and managing cash flow
FinTech, Grants & Funding Opportunities
A workshop that covers budgeting apps, online investing platforms, digital wallets, and cybersecurity. In addition, grant & funding topics to understanding eligibility, applications, and compliance requirements.

Technology Training
Tech Training Workshops are designed to introduce participants to essential digital and coding skills through interactive learning, game-based platforms, and guided tutorials. Using tools such as Code Academy and other coding apps, participants will build practical technology knowledge in an engaging, hands-on environment that makes learning to code accessible, fun, and relevant to real-world applications.
Interactive Learning
Interactive learning engages participants through hands-on activities that encourage active participation rather than passive observation. Learners practice real-time problem-solving by writing code, experimenting with digital tools, and collaborating with peers. This approach reinforces understanding by allowing participants to immediately apply new concepts, ask questions, receive feedback, and build confidence as they learn by doing.
Game-Based Learning
Game-based learning uses challenges, levels, rewards, and storytelling to make technology training engaging and motivating. By turning coding concepts into games and interactive tasks, participants develop logical thinking, creativity, and persistence in a low-pressure environment. This method promotes curiosity and resilience, helping learners overcome obstacles while reinforcing coding fundamentals through play and achievement.
Guided Learning Tutorials
Guided tutorials provide structured, step-by-step instruction that supports learners at every stage of the learning process. Participants follow clear lessons that explain concepts, demonstrate examples, and gradually increase in complexity. With guided support, learners can progress at their own pace while building a strong foundation, ensuring comprehension, consistency, and measurable skill development.
